SCHEDULING APPOINTMENTS<\/strong><\/h5>

Prior to your inital appointment, please fill out your new client forms online, at least 48 hours prior to your scheduled appointment. New client forms will be emailed to you through your client portal once your appointment has been confirmed.<\/p>

PAYMENTS<\/strong><\/h5>

The total cost of the appointment is due at time of service . An active credit card must be kept on file.<\/p>

TIMELINESS<\/strong><\/h5>

I reserve the right to cancel your appointment if you are more than 10 minutes late.\u00a0 Appointment sessions will end on time.<\/p>

REFILL REQUEST<\/strong><\/h5>

Medications will be refilled at each appointment.\u00a0 Refill prescriptions will be called in for current patients who maintain thier regulary scheduled visits.\u00a0 Refill requests will have a 24-48 hour turn around time.\u00a0 Emergency refill request are not available. Prescriptions will not be called in after operating buisness hours or during holidays.\u00a0 Prescriptions will not be called in for anyone who repeatedly misses their scheduled appointments.\u00a0<\/p>

CANCELLATATION POLICY<\/strong><\/h5>

If you miss your appointment, you will be charged 50% of the cash pay rate for the appoinment. To ensure you do not get charged this fee, please cancel your appointments 24 hours, or earlier , ahead of your start time.\u00a0<\/p>

CLIENT PORTAL<\/strong><\/h5>

After you have completed your pre-screen questionnaire and offered an appointment, you will be sent a link to my secure, online client portal.\u00a0 There you can schedule or cancel follow up appointments, request refills,report mild to moderate side effects\/symptoms and provide non-urgent Mental correspondance.\u00a0<\/p>

CONTROLLED MEDICINES<\/strong><\/h5>

These medications will not be refilled if appointments are missed. They will not be refilled in between appointments . This includes medications for ADHD.<\/p>

Chronic use of benzodiazepines is contranindicated.\u00a0 If prescribed a benzodiazepine, a formalized plan will be in place to discuss time frame use and treatment options for non-controlled medications to be considered.\u00a0<\/p>
